  Music Therapy and Lessons Registration

Congratulations on your decision to deepen your relationship with music through our programs here at Oak Park Music Therapy.  We are a very DIVERSE group, serving traditional guitar, bass, violin, and ukulele students, to learners needing adaptations and creative approaches, to bedside services for clients unable to leave the comforts of home or hospital.  Our music education and music therapy services are offered in homes, community settings, hospitals, schools and via TELEHEALTH ONLINE.

We are delighted that you are joining us on this journey of making music together!

Here's your next steps:

1. Take a few moments to read our Studio Policies section below - you will be asked to agree with these items before registering yourself or your child.

2. Then please fill out the registration form below completely and thoroughly.  You can give or restrict permission for us to use photos of yourself/child/site for promotional and educational purposes.  Choose a username and password and you will be on your way.

3. Once you have registered, you will get an email describing how to login to the system. The next time you come to the site, you will go to the Login page to access your records.

4. If you are registering for music therapy assessment and possible services, you can print and complete the documents listed in the Music Therapy Docs tab (coming soon) and have them ready for your first appointment once scheduled.  For music lesson students, there are no additional forms needed.

We hope this site will be a benefit to you in making it easy to schedule or reschedule sessions, have access to our calendar of classes and events, print out forms and policies, easily view your invoice, and pay your bill.  Payment options are available (check, online banking, Venmo, or PayPal) for your convenience, and we may offer you some feedback on how sessions are going through the email interface, as well as send messages about the lesson/session.

Please review these policies.  Registering for services indicates you agree to them prior to commencing services.

FINANCIAL RELEASE

I agree to pay the fees for my child/self/organization to participate in Oak Park Music Therapy services.  Payment may be made by cash, check, online through your bank, or through PayPal. Sessions missed without 24hr notice will be billed in full. Oak Park Music Therapy reserves the right to cancel a class should the minimum not be met for group.  Refunds will be processed if a class is canceled.  No refunds are available for missed group sessions due to illness, late registration, or otherwise.  

MEDICAL RELEASE

In the unlikely event that my I or child becomes ill or is injured and I, or the authorized physician named on the registration form cannot be immediately contacted at the time of the emergency, and if in the judgment of the staff/agents of Oak Park Music Therapy deem immediate treatment is necessary, I authorize and direct the staff/agents to send me or my child (accompanied by an adult) to the hospital or physician most easily accessible. I release Oak Park Music Therapy, its staff/agents/contractors, from any claim of liability in connection herewith.

SESSION/LESSON RELEASE

I will make every effort to ensure that I/my child arrives on time and will be picked up regularly in a timely manner from sessions/lessons on site at the Oak Park Music Therapy studios.  I understand that sessions held in my / my child's home (or other offsite location) requires that at least one adult or responsible party remain in the home during the session.  I do not hold the staff/agents/contractors of Oak Park Music Therapy responsible in any way for me or my child outside of the identified start or ending time of class, sessions, lessons or events.

I understand the above and by registering for services, I agree with such conditions.

Welcome to the registration area for our music therapy services, music lessons, and group classes in the community or school. Please consider the term "parent" to mean a guardian, loved one, organization, or responsible party who will be paying the fees for services. An "adult student" is considered a person who is receiving services and is responsible for payment themselves, regardless of age. A "child" or "child student" is a student or client who is being supported by the "parent". This includes adult students/clients whose bills are being paid by another party or organization.
